Inputs and expected output

Collect the final research question, design, main result, most important scope boundary, target-journal instructions, and terminology readers actually use. The output should be three candidates: descriptive, declarative, and question-based. Choose the one that most accurately matches the evidence and journal convention.

Draft from a full sentence

Write one plain sentence describing the paper: “In a two-hospital observational cohort, greater continuity of follow-up was associated with fewer unplanned readmissions, with substantial uncertainty in the smallest subgroup.”

Then compress without deleting what controls interpretation:

  • Descriptive: “Continuity of follow-up and unplanned readmission in a two-hospital cohort”
  • Declarative: “Greater continuity of follow-up is associated with fewer unplanned readmissions in a two-hospital cohort”
  • Question: “Is continuity of follow-up associated with unplanned readmission?”

The declarative title carries the result but must preserve “associated.” Replacing it with “prevents” would exceed the observational design. The descriptive title is safer when the estimate is complex, mixed, or secondary.

Choose a title type deliberately

Use a descriptive title when the main value is the study object, method, dataset, or nuanced relationship. Use a declarative title when one primary result is well supported and the journal accepts result-bearing titles. Use a question title only when the paper actually answers that question and the form improves clarity rather than suspense.

Do not add a colon merely to sound academic. A subtitle earns its place when it supplies design or scope information the first phrase cannot hold cleanly.

Test accuracy, retrieval, and reader expectation

Check every important word against the manuscript. Does “randomized” match allocation? Does “national” match sampling? Does “long-term” match follow-up? Does “mechanism” have mechanistic evidence? Then compare the title with phrases used in relevant literature without copying another paper's wording.

Submit the title if; hold if

Submit if the title and abstract make the same evidence-bounded promise, the design label prevents a likely causal or scope error, and every strong verb is supported by the primary analysis. Hold if novelty adjectives replace the contribution, an acronym hides the subject from adjacent readers, or the title generalizes beyond the sampled population, material, or setting.

We compare each candidate against the abstract, main figure, and conclusion because title accuracy is a cross-surface property. A title can sound excellent in isolation while mislabeling the design or elevating a secondary finding. Use the abstract-writing guide to reconcile the question, result, and boundary before the title is locked.

Also test the title with one informed reader outside the immediate project. Ask what study design, population, and strength of conclusion they expect from the words alone. A mismatch is evidence that the title needs a boundary or a less forceful verb, not evidence that the reader needs more background.
