Worked question and first search
Example question: Among university researchers, does structured pre-submission manuscript feedback improve reporting completeness before journal submission?
A first concept search could be:

Test each concept block separately, confirm that several known relevant papers appear, and inspect the terms those records use. If the intervention is described as “author support” or “manuscript assessment,” add those terms because the evidence showed a vocabulary gap, not because a longer string looks more rigorous.
Translate, do not paste
The concepts should remain stable across databases; the syntax should not. PubMed may use MeSH and field tags. Scopus and Web of Science use different title/abstract/keyword fields and proximity operators. A psychology database may supply its own thesaurus.
For every platform, save:
- database and vendor platform;
- full search string exactly as run;
- controlled-vocabulary terms and exploded headings;
- date and any date, language, document-type, or human-study filters;
- number of records returned and exported;
- file name or search-history identifier.
If a reviewer cannot reconstruct the search from the manuscript and supplement, the method is not yet ready.
Test sensitivity with known papers
Choose three to five papers that should appear because they directly answer the question or anchor the field. Run the draft search. A missing paper is a diagnostic:

Sensitivity testing does not prove completeness, but it exposes obvious blind spots before screening begins.
Keep the search and screening ledgers separate
The search ledger records how records were found. The screening ledger records why records were included or excluded. Mixing them leads to a common methods defect: exclusion decisions quietly reshape the query after results are known.
Deduplicate exported records using DOI first, then stable identifiers and title-author-year matching. Preserve the raw exports. If software makes automatic merges, spot-check them; two reports from one study may be related but not duplicates.
Add supplementary routes deliberately
Depending on the question, add backward reference checking, forward citation searching, trial or protocol registries, grey-literature sources, preprints, conference records, and contact with experts. Name each route and its date. “Reference lists were checked” is too vague when the route materially affects coverage.

Write the methods paragraph from the ledger
A credible methods paragraph names the databases and platforms, search dates, broad concept structure, eligibility boundary, deduplication and screening approach, and where the full strategies can be inspected. It does not call a search “systematic” simply because Boolean operators were used.
Before submission, compare every number in the flow diagram, methods, supplement, and results. A manuscript methods check is most useful when the exact search strings and screening record are available.

Think twice if the search becomes longer but not clearer
More synonyms do not automatically increase useful sensitivity. Hold the strategy when a new term adds mostly irrelevant records, changes the population, or smuggles an outcome into a question that should remain outcome-neutral. Record the test result, then keep, revise, or remove the term with a reason.
Check concept blocks independently before combining them. A zero-result final string can hide one broken block, an unsupported field code, or a subject heading unavailable on that platform. A very large set can hide a generic term that overwhelms every other constraint. Save the count after each block and after each major revision so the cause remains traceable.
Do not repair a known database-coverage gap with citation chasing alone. Citation routes are valuable supplementary searches, but they are shaped by the seed set and publication age. When the question requires a subject database, either add it or state the limitation explicitly.
