Quick answer: Write a narrative review when you're synthesizing theory, tracing how a field's understanding evolved, or offering expert perspective across a body of work too heterogeneous to pool. Write a systematic review when you need a reproducible, bias-controlled answer to a specific, narrow question. What a narrative review can do that a systematic review can't

Cover ground a systematic search would exclude. A narrative review on, say, the history of a theoretical model can draw on foundational papers, adjacent-field work, and grey literature that a systematic search's strict eligibility criteria would filter out. The breadth is a feature, not a shortcut.

Carry genuine expert judgment. The value of many narrative reviews is exactly that an experienced researcher is telling you what matters and why, in a way a mechanical search-and-extract process cannot replicate. This is a real, defensible reason to choose narrative over systematic, not a consolation prize.

Move faster on a broad question. A narrative review doesn't require registering a protocol or running risk-of-bias assessment on every included study, so it can be produced in a fraction of the time a systematic review needs for a comparably broad topic. Think twice before choosing narrative purely for speed, though: if the underlying question is actually narrow and answerable, a systematic review gives reviewers something a narrative review structurally cannot, an auditable process they can check independently of how convincing the prose sounds.

Where narrative reviews are vulnerable

Selection bias with no audit trail. This is the specific, testable failure pattern: because there's no documented search strategy, a reader has no way to check whether the cited literature represents the field or represents a curated set that happens to support the author's existing view. Two experts writing narrative reviews on the same topic can reach opposite conclusions, and neither reader can tell which selection was more representative. The field has partly acknowledged this gap: SANRA, a six-item scale published in 2019, exists specifically to give narrative reviews a quality-assessment tool that systematic reviews already had through PRISMA.

Treating "review article" as a single category during submission. Many journals score narrative and systematic reviews against different criteria and, in some cases, route them to different sections or editors. Submitting a narrative review under a cover letter that describes systematic methodology invites an editor to check the methods section against a checklist it was never built to satisfy.

A worked example

Consider two review articles on the same broad topic: chronic stress and cardiovascular disease. The first asks "how has the field's understanding of the stress-cardiovascular link evolved from the 1970s to today, and what theoretical models have competed for explanatory power." That's a narrative review question. It requires synthesizing decades of theory, including foundational papers that wouldn't survive a modern systematic search's inclusion criteria, and it benefits from an author who can weigh which theoretical frameworks actually shaped the field versus which were influential briefly and then abandoned.

The second asks "does a specific stress-reduction intervention lower measured cardiovascular risk markers in adults with hypertension." That's a systematic review question: a defined population, a defined intervention, measurable outcomes, and a literature specific enough that a documented search could plausibly retrieve most of the relevant trials.

The trouble starts when someone answers the narrow, intervention-specific question but writes the manuscript with a narrative review's looser citation habits: no eligibility criteria, no search dates, no account of why some studies appear and others don't. A reviewer working in that intervention space notices the gap immediately, because familiar trials are missing and nothing in the methods explains the omission.

How editors read the difference

An editor triaging a narrative review is mostly asking whether the author has the standing and clarity to make the argument credible: is this a recognized voice in the field, does the piece cite the foundational and the recent work fairly, and does the argument hold together. An editor triaging a systematic review is asking a different question first: can I trust the process that produced this evidence set, before I even evaluate what it concludes.

That difference in triage is why a systematic review with a thin or undocumented search gets flagged even when its conclusions sound reasonable, and why a narrative review with a genuinely novel synthesis can succeed even though its citation list would never pass a systematic eligibility audit. Neither standard is looser than the other; they're evaluating different things.

A middle path: the semi-systematic review

Some questions don't cleanly fit either category. A semi-systematic (or critical) review runs a documented, reproducible search but skips formal risk-of-bias scoring and quantitative pooling, then synthesizes findings narratively. It is a legitimate hybrid when the literature is too methodologically diverse to pool but a fully undocumented search would still leave the review's coverage impossible to evaluate. State explicitly in the methods which systematic elements were used and which were deliberately skipped, and why. Where a narrative review falls short is offering an auditable, bias-controlled answer to a narrow question. Where a systematic review falls short is capturing the kind of cross-field theoretical synthesis an experienced author brings to a broader topic. Know when neither fits: a short background section citing a handful of foundational papers doesn't need review-article structure of either kind.

Match the manuscript's language to the actual review type you ran.
